Chronic Pelvic Pain
The defining feature of CPPS. Pain may be constant or intermittent, dull or sharp, localised or diffuse. Many men describe it as an ache that is always present in the background.
Chronic Pelvic Pain Syndrome (CPPS) is one of the most common urological conditions in men, yet it is widely misunderstood and frequently undertreated. A combination of Focused Shockwave Therapy and specialist pelvic floor physiotherapy targets the root causes of your pain, not just the symptoms.
Understanding the condition
Chronic Pelvic Pain Syndrome (CPPS), also known as chronic prostatitis Category III, is persistent pain or discomfort in the pelvic region lasting at least three months, without evidence of a bacterial infection. The pain may be felt in the perineum, lower abdomen, testicles, tip of the penis, or lower back, and is frequently accompanied by urinary and sexual symptoms.
CPPS is not “all in your head” and it is not something you simply have to live with. It is a recognised physiological condition with established, evidence-based treatments that deliver meaningful relief.
The condition is multifactorial. In most men, pelvic floor muscle dysfunction, nerve hypersensitivity, and localised inflammation interact to produce and maintain symptoms. This is why the most effective approach combines shockwave therapy to address tissue-level inflammation and nerve sensitisation, alongside pelvic floor physiotherapy to release muscular tension and correct dysfunction.
You do not need a GP referral to visit an ED Clinic. Our specialists can assess you directly.

Research consistently shows that CPPS affects quality of life to a degree comparable to conditions such as diabetes and congestive heart failure. Its impact rarely stays physical.
The defining feature of CPPS. Pain may be constant or intermittent, dull or sharp, localised or diffuse. Many men describe it as an ache that is always present in the background.
Urgency, frequency, a weak or interrupted stream, and discomfort when passing urine are all common. These symptoms are often mistaken for a prostate problem or UTI.
A 2025 meta-analysis of over 20,000 men found that 59% with CPPS experience sexual dysfunction, including erectile dysfunction in 34% and premature ejaculation in 35%.
Anxiety affects up to 73% of men with CPPS and depression up to 52%, rates far above the general population. Chronic pain and uncertainty about diagnosis drive a cycle of distress that worsens symptoms.
Around 25% of men with CPPS report loss of working days. Sitting for extended periods, a significant trigger for many, makes desk-based work particularly difficult to sustain.
Pain during or after sex, reduced libido, and the psychological weight of a chronic condition all take a toll on intimate relationships. Many men find this harder to talk about than the pain itself.
The ED Clinics approach
CPPS responds best to a multimodal approach that addresses both the tissue-level and neuromuscular drivers of pain. At ED Clinics, we combine two evidence-based treatments in a coordinated programme tailored to your presentation.
Focused Shockwave Therapy delivers targeted acoustic waves to the prostate region and pelvic floor, reducing inflammation, desensitising overactive nerves, and stimulating tissue repair. A 2024 systematic review and meta-analysis published in Neurourology and Urodynamics confirmed shockwave therapy produces significant pain relief and quality of life improvements in CP/CPPS, and recommended it as a safe, non-invasive treatment option.
Pelvic Floor Physiotherapy addresses the muscular component by releasing trigger points, reducing hypertonicity, and retraining dysfunctional movement patterns that perpetuate pain. Clinical guidelines and peer-reviewed studies confirm pelvic floor physiotherapy as an effective treatment for CPPS with pelvic floor dysfunction.

A word about our clinics
We understand that attending a clinic for a pelvic pain condition can feel daunting. Our clinics are physiotherapy practices with specialist expertise in men’s pelvic health, not sexual health clinics. What appears on your bank statement, appointment communications, and clinic signage reflects the physiotherapy practice, not your specific reason for attending.
Our practitioners are experienced in treating CPPS and approach every patient with discretion and without judgement. Many of the men we see have spent years being told their pain has no identifiable cause. We take it seriously from the outset.
